Talahi Home Health, LLC is contracted to provide health services at Nature's Point - Assisted Living. Talahi Home Health, LLC is licensed by the Minnesota Department of Health as an Assisted Living Home Care Provider. Talahi Home Health staff is on site 24-hours a day to respond to your anticipated and unexpected needs.

Health Service Package Includes:

• RN Review and Admission to Home Care.
  
• 24-Hour Emergency Response System by Staff Associate.
  (Charges apply if hands on care provided)
  
• 24-Hour Phone Receptionist.
  
• 24-Hour Emergency Response from Talahi Care Center.
  
• Licensed Staff available On-Call Support 24-hours a day.
  
• Transportation scheduling and appointment arranging.
  
• RN Evaluation at least annually.
  
• RN Maintenance of clinical record information.
  
• RN On-Site (as scheduled)
